Omaha Fire investigators say careless smoking caused an accidental fire late Wednesday night in an apartment near North 100th Plaza and Kansas Plaza, southeast of I-680 and Military Road.
Firefighters pulled up just after 11 p.m. to find smoke coming from an apartment, though it was determined that the fire had burned itself out.
There were no injuries reported;
