Creighton University officials say an IT issue led to a data loss of electronic patient records on June 18. This data lost included prescription and refill history, and insurance information for all customers.
University officials emphasize that no patient records were stolen as this was a loss of data, not a breach. The database will need to be rebuilt, requiring all patient data to be re-entered and new prescriptions obtained from physicians.
Patients will need to contact their doctor to get new prescriptions written or called in to the pharmacy for current medications. You can ask the pharmacy to contact your physician for you by calling the pharmacy at 402.449.4560.
If you receive your prescriptions from the Creighton University Pharmacy you will need to complete a patient demographic form , fill it out and mail it to Creighton University Campus Pharmacy, 2412 Cuming St., Suite 201, Omaha, NE 68131. Patients also can fax the completed form to (402) 449-4531.
